This form must be uploaded or returned completed to us by no later than close of business on 20 October 2023, in order to have effect. If we do not receive the completed form back from you by that date then it will be assumed by the Department of Infrastructure, Planning and Logistics that you do NOT wish to have an inspection carried out.
